At The Hallen School, we are dedicated to ensuring that every student is equipped with the skills, knowledge, and support they need to transition smoothly into the next chapter of their lives. Our comprehensive transition process helps students explore their interests, set goals, and prepare for post-graduation opportunities, including higher education, vocational training, and independent living.
Transition Process
The transition journey begins when a student turns 14 years old, as part of their Individualized Education Program (IEP). This process is a collaborative effort involving the student, their family, teachers, school counselors, and related service providers such as speech-language, occupational, and physical therapists.
Key Elements of the Transition Process:
Identifying Goals: Students are encouraged to think about their interests, strengths, and career aspirations.
Creating a Plan: Post-secondary goals are documented in the IEP, outlining the steps needed to achieve them.
Skill Development: Academic, social, and life skills are emphasized to prepare students for independence.
The IEP acts as a roadmap, detailing the student’s goals, accommodations, and services needed to support their transition journey.

Connecting with resources
Prior to graduation, students and their families are introduced to local and state-wide agencies that provide ongoing support. These organizations help bridge the gap between school and post-graduation opportunities.
ACCES-VR
(Adult Career and Continuing Education Services – Vocational Rehabilitation)
- Provides services for individuals aged 14-22 to achieve and maintain employment.
- Offers training, education, and career development to support independent living.
OPWDD
(Office for People with Developmental Disabilities)
- Offers services for individuals with intellectual and developmental disabilities.
- Supports students with IEP classifications such as Intellectual Disability (ID), Multiple Disabilities (MD), or Autism.
Pre-Employment Transition Services
(Pre-ETS)
- Designed for students aged 14 and older who may be eligible for ACCES-VR.
- Focuses on career exploration, job readiness, and skills development.
Bronx Transition and College Access Center
(TCAC)
- Provides guidance and resources for college and career planning.

Summer Internships and Employment:
Students gain valuable hands-on experience through internships and summer employment opportunities. These experiences allow students to explore potential careers, build resumes, and develop workplace skills in a supportive environment.
School Presentations and Workshops:
Throughout the year, our Transition Coordinator and Vocation Coordinator & Community Business Liaison organize:
- Presentations introducing juniors and seniors to available services.
- Tours of colleges and technical schools to help students envision their next steps.
- Participation in job fairs and career workshops.
Parents and guardians are notified of these events and are encouraged to participate, ensuring a strong support system for students.

We understand that the transition process can be overwhelming for families. That’s why our counselors and Transition Coordinator are here to assist.
College Applications: Guidance on completing applications and preparing required documents.
Financial Aid and Scholarships: Support with navigating financial aid forms and identifying scholarships.
Job Training Opportunities: Connecting families to programs that align with their child’s goals for the future.
